Insurance
I work with a variety of insurance companies. If you would like my billing department to check your benefits for acupuncture, please send me an email with your name, phone number, address, date of birth, and the insurance ID number, group number, and name of insurance company. You can also send a photo of your insurance card along with your date of birth and address if that's easier.If you will be using your insurance coverage to pay for acupuncture treatments, we must verify your coverage before your first visit or you will be asked to pay the out of pocket rate until acupuncture coverage is confirmed. Missed visits or late cancellations cannot be billed to insurance.Flex-spending and health-savings accounts reimburse acupuncture costs.
Fees
For patients not utilizing insurance coverage, the rates with same-day payment discount are:Initial treatment -$130 (approximately 1.5 hours)Follow-up treatments - $85 (1 hour)Cash, check, and credit cards are accepted. Payment is due at the time of service.Herbal medicine, if prescribed, is approximately $18-20/week.
Cancellation Policy
Your appointment time is reserved for you. We request 24-hour notice for cancellation of appointments. Missed appointments or late cancellations will be assessed a fee.
